Children's Hospital in Omaha, Neb. developed the Hand in Hand Program: A Palliative Care Solution for patients of Children’s
Hospital.
This program addresses the physical, emotional, social and spiritual needs of children with life limiting or life-threatening illnesses as well as their families. Utilizing a multidisciplinary experienced team, it seeks to prevent and relieve distress and support the best possible quality of life for the patients and families. The Hand In Hand Program is the first pediatric palliative care model in Nebraska and can be reproduced throughout the state.

Thayer County Health Services in Hebron, Neb. developed and implemented a sustainable interoperable system to improve the flow of medical information. The goal of Thayer County Health Services’ quality initiative was to focus on developing and implementing a sustainable interoperable system to improve the flow of medical information along a continuum of care. By creating an electronic network between health care providers and their partners, decisions can be made based upon current patient data and information. This network has lead to an increase in patient safety by reducing the number of medication errors and near misses, improving medication reconciliation through electronic prescribing, and increasing efficiency of patient diagnosis and treatment.
